12.4 Express Enrollment Application Process

This section details the Express Enrollment application process. Users should keep in mind that the application is dynamic depending on the answers to questions, so the screenshots in this section may not exactly reflect the appearance of the application pages.

Reminder: APP pages contain a link to change the on-screen text to Spanish and a Help button, which, when clicked, will open a new page that explains more about what is being asked and how questions should be answered.

12.4.1 Location Selection Page

Once a security administrator, location administrator, or user logs in to ACCESS, the Location Selection page will display if that individual is associated with more than one Express Enrollment location. The individual must select the appropriate location before continuing.

Note: Administrators and users will only be able to search for Express Enrollment applications that were started or submitted under the location selected.

 

Example Jamie is a user who is associated with Location A and Location B. After Jamie logs into ACCESS, he selects Location A. Jamie should only make presumptive eligibility determinations for individuals he is working with at Location A. Jamie will only be able to search for past Express Enrollment applications that were started or submitted under Location A. If Jamie wants to search for past Express Enrollment applications started or submitted under Location B, he will need to log out of ACCESS, re-login and select Location B.

If an individual is associated with only one Express Enrollment location, the Location Selection page will not display and the individual will instead be taken directly to the ACCESS Landing Page.

12.4.2 Landing Page

The Landing page will display appropriate options based on the logged-in individual’s security profile. The next page the individual is taken to will depend on the action he or she chooses from this list.

12.4.2.1 Submit Express Enrollment Applications

Note: Although security administrators and users can both submit Express Enrollment applications, this section will use the term “user” to refer to both types of user.

To start an Express Enrollment application to temporarily enroll someone in BadgerCare Plus or Family Planning Only Services, the user must select the appropriate option on the Landing page and click on the Next button at the bottom of the page. The options available in this section are based on the user’s security profile so all of the options described in this section may not appear on the Landing Page.

Multiple individuals from the same household who want Express Enrollment in BadgerCare Plus can be included on the same Express Enrollment for BadgerCare Plus application. However, only one individual can be included on an Express Enrollment for Family Planning Only Services application, even if the individual is a minor, is married, and/or has children. If multiple individuals in the same household want Express Enrollment for Family Planning Only Services, users must complete separate Express Enrollment for Family Planning Only Services applications for each individual.

To search for applications or manage his or her account, the user must select the appropriate option in the Search for Applications/Manage My Account section and click on the “Next” button at the bottom of the page.

The options available to search in this section are based on the logged-in user’s security profile.

Only applications from the current location (as selected on the Location Selection page if the logged-in user is associated with multiple locations) will be displayed in the search results. To search for applications submitted at a different location, the user must log out and re-log in to ACCESS, and select the other location.

12.4.3 Individual Information Page

Once the user has chosen to submit an application, the Individual Information page will display. This page collects information about the individual(s) who is applying for Express Enrollment.

For each individual in the household who wants to temporarily enroll in BadgerCare Plus or Family Planning Only Services through the Express Enrollment process, the user must enter the individual’s first name and last name. The user should enter the individual’s full legal name. If the individual has two last names, the user should enter them both in the last name box. If the name doesn’t fit in the boxes, the user should enter as much of the name as possible.

The user may add individuals for the household by clicking on the “Add” button at the bottom of the Individual Information page. If the user needs to delete an individual, he or she should click the “Delete” button on the right side of the entry for that individual.

Reminder: Only one individual can be included on an Express Enrollment for Family Planning Only Services application, even if the individual is a minor, is married, and/or has children. If multiple individuals in the same household want Express Enrollment for Family Planning Only Services, the user must complete separate Express Enrollment for Family Planning Only Services applications for each individual.

Depending on the information entered for an individual, additional questions may display. The following information may be requested:

  1. Current enrollment in Wisconsin Medicaid or BadgerCare Plus
    The individual is not eligible for temporary enrollment if he or she is already receiving ongoing benefits. See Section 12.2.1.1 Current Enrollment in Ongoing Benefits for more information on the situations where this applies.
  2. Parent or caretaker status
    1. Stepfather or stepmother.
    2. Natural full brother or sister, legally adopted, half- or stepbrother or sister.
    3. Grandfather or grandmother, uncle or aunt, first cousin, nephew or niece, or any preceding generation denoted by the prefix grand-, great-, or great-great, and including those through adoption.
    4. Spouse of any of the above even after the marriage ends by death, divorce, or separation. A spouse is a person recognized by Wisconsin law as another person’s legal husband or wife. Wisconsin does not recognize common law marriage.
      Once a child marries, he or she can no longer be considered under the care of a caretaker relative.
    5. A parent is any natural, legally adoptive, or step mother or father of a child under age 19. In order for an individual to be considered a parent of a child under age 19, the child must live with that individual at least 40 percent of the time.
    6. A caretaker is a non-legally responsible relative of a child under his or her care, meaning the caretaker exercises primary responsibility for the child’s care and control, including making plans for the child. To be considered a caretaker relative of a child in the home, an individual must have a qualifying relationship to the child under age 19, and the child must be under the care of the individual. Qualifying relationships for caretaker relatives consist of the following:
  3. Citizenship or immigration status
    See Section 12.2.2.3 U.S. Citizen or Qualifying Immigrant for more information about citizenship or immigration status requirements.
  4. Previous Express Enrollment
    See Section 12.2.1.2 Temporary Enrollment Within the Last 12 Months for more information about previous Express Enrollment.
  5. Former Foster Care Youth status
    A Former Foster Care Youth is an individual age 18 through 25 who was receiving foster care, subsidized guardianship, or court-ordered kinship care on the date that he or she turned 18. It does not matter what state he or she was residing in when he or she turned 18.
  6. Currently receiving Medicare Part A or Part B
    If an adult without dependent children in the home is currently receiving Medicare Part A or Part B, he or she is not eligible to temporarily enroll in BadgerCare Plus through the Express Enrollment process.
  7. Pregnancy status
    If an individual is currently pregnant, additional questions about the number of babies expected and the expected delivery date will display. If the number of expected babies is unknown, the user should enter 1. If the exact due date is not known, the user should provide the best estimate based on information provided by the individual.

When the user clicks the “Save and Exit” button, the data the user entered will be saved and the user will be taken back to the Landing page.

The "Next" button will take the user to the Contact Information page.

12.4.4 Contact Information Page

The Contact Information page collects information about the primary contact for the Express Enrollment application, where the individual(s) lives and the best method to contact him or her.

Primary Contact Person

For Express Enrollment for BadgerCare Plus, the primary contact person depends on who is applying. If the person is under age 18 and is not living independently, he or she must have a parent, caretaker, or guardian apply for Express Enrollment for BadgerCare Plus for him or her.

For Express Enrollment for Family Planning Only Services, there is always only one individual on the application, so this individual will be designated as the primary contact person. A minor applying for Express Enrollment for Family Planning Only Services does not need a parent, caretaker, or guardian to apply for him or her, even if he or she is not living independently.

Express Enrollment application including individuals age 18 or older and minors

If the Express Enrollment application includes at least one individual age 18 or older and at least one minor, a drop-down menu will be displayed with the names of all of the individuals on the application who are age 18 or older and an additional “Other Parent/Caretaker” option. The user must choose one individual from the drop-down menu as the primary contact person. If another person who is in the household but not on the Express Enrollment application wants to be the primary contact person, the user should choose the “Other Parent/Caretaker” option. If the “Other Parent/Caretaker” option is selected, the user must enter the following information for that individual into the Express Enrollment application:

Express Enrollment application for only minors

If the Express Enrollment application includes only people under age 18, the user must indicate for each person whether or not the person is living independently.

If all people on the application are under age 18 and living independently, the oldest person will automatically be designated as the primary contact person.

If the people under age 18 on the application are not living independently, the user must enter the following information of the parent, guardian or other caretaker who is applying for them:

Home Address

The user must first enter the street address where the individuals on the application live. If they are homeless, the user still needs to enter an address. This can be the address where they are currently staying (for example, with a friend, family member, shelter or other organization). If they are homeless and do not have an address where they are currently staying, the user can enter the address of the IM agency of the county where the individuals reside. IM agency addresses can be found at dhs.wisconsin.gov/forwardhealth/imagency/index.htm.

After entering the street address, the user must choose the county where the individuals on the application live.

If the individuals live on tribal lands, the user should select the appropriate tribe from the dropdown menu. If the individuals do not live on tribal lands, this field can be left blank.

Reminder Only Wisconsin residents are eligible to temporarily enroll in BadgerCare Plus or Family Planning Only Services through Express Enrollment.

Mailing Address

If the mailing address is different than the home address, the user should check the “Different than Home” box and enter the complete mailing address, including ZIP code.

If no mailing address is entered, notices will be sent to the home address. If a mailing address is entered, notices will be sent to the mailing address.

If the individuals on the application are homeless, the user should enter the most reliable address where the individuals can receive mail about their enrollment.

If homeless individuals have no alternative mailing address, the user should not check the “Different than Home” box. If the home address entered is the address of the IM agency, the user must instruct the individuals to contact the IM agency for information on picking up their notices.

Contact Information

The user must enter any information the primary contact person has provided on how to get in touch with him or her. A “Message Phone” is a phone number where the primary contact person can be reached or where a message can be left with him or her. This could be voicemail or the phone number of an organization, family member or friend. If there is no contact information, these fields can be left blank.

The “Back” button will take the user back to the Individual Information page.

The “Save and Exit” button will save the data entered and take the user back to the Landing page.

The “Next” button will take the user to the Income Details page.

12.4.5 Income Details Page

The Income Details page collects information about the household income.

12.4.5.1 Household Size

For Express Enrollment for BadgerCare Plus, the household size must include all members of the household, even if they are not requesting Express Enrollment and are not listed on the application. Household members include:

For Express Enrollment for Family Planning Only Services, the household size can only be “1” regardless of whether the individual is under age 18, married and/or has children, so this field will be automatically filled with “1.”

12.4.5.2 Monthly Earned Income

For Express Enrollment for BadgerCare Plus, earned income from all household members listed in 12.4.5.1 should be reported, even if they are not requesting Express Enrollment and are not listed on the application.

For Express Enrollment for Family Planning Only Services, only the earned income of the individual on the application should be reported.

Earned income is income resulting from performing a job or providing a service. Earned income includes commissions, tips, salaries, wages and self-employment. To calculate the amount of the monthly earned income, the user must first determine how the employee is paid:

12.4.5.3 Monthly Other Income

For Express Enrollment for BadgerCare Plus, other monthly income from all household members listed in 12.4.5.1 should be reported, even if they are not requesting Express Enrollment and are not listed on the application.

For Express Enrollment for Family Planning Only Services, only the other monthly income of the individual on the application should be reported.

Other monthly income includes taxable income from a source other than a job or self-employment. For example:

The Back button will take you back to the Contact Information page.

The Save and Exit button will save the data and take you to the Landing Page.

12.4.6 Provider Signature Page

The Provider Signature page collects the electronic signature of the user completing the Express Enrollment application.

The user must click on the link to the “Rights and Responsibilities” in the Provider Signature section and review the document with the primary contact person.

The user's and the primary contact person’s name will be listed in the Authorization Box section. After reading the statement in the Authorization Box section, the user must enter his or her signature by checking the box in front of the statement.

The “Back” button will take the user back to the Income Details page.

The “Next” button will save the data and take the user to the Client Signature page.

12.4.7 Client Signature Page

The Client Signature page displays a summary of all the information entered on the Express Enrollment application. The user must ask the primary contact person to review all of the information to make sure it is accurate.

The primary contact person’s name will be listed in the Authorization Box section. Once the primary contact person has reviewed the summary and read the statement in the Authorization Box section, the primary contact person should check the box verifying that he or she understands the eligibility results and sign the application by typing his or her first and last name in the appropriate boxes.

Reminder: The primary contact person must sign the application in person. Telephonic signatures are not allowed for Express Enrollment.

The “Back” button will take the user back to the Provider Signature page.

The “Submit” button will complete the application and take the user to the Results page. Once the “Submit” button is selected, the Express Enrollment application status will change from In-progress to Submitted.

Once the “Submit” button is clicked, APP will run a systematic check with ForwardHealth to see if any of the following apply:

Users may see the following message while APP is running the systematic check with ForwardHealth:

If unable to successfully connect with ForwardHealth, APP will use only the information from the Express Enrollment application to make the presumptive eligibility determination.

12.4.8 Results Page

The Results page displays the results of the Express Enrollment application and confirms that the application has been completed. This page includes buttons for printing the Eligibility Notice and the Application Summary, both of which must be printed and provided to the applicant.

The “Exit” button will take the user back to the Landing page.

12.4.8.1 Eligibility Notice

The Eligibility Notice is available in both English and Spanish. The user should click on the appropriate button to view and print the PDF in the desired language. The user must print the Eligibility Notice and give it to the primary contact person. If the applicant is not eligible, the notice will state the reason(s).

12.4.8.1.1 Temporary Identification Card

If eligible for temporary enrollment, the Eligibility Notice will include a temporary identification (ID) card for each applicant eligible for Express Enrollment in BadgerCare Plus or Family Planning Only Services . The temporary ID card is valid for 14 calendar days and can be used until a permanent ForwardHealth ID card is mailed to the mailing address indicated on the Express Enrollment application. The permanent ForwardHealth ID Card will be mailed to the applicant within 3-5 business days, if the applicant has not already been issued a ForwardHealth ID card. If the applicant has been issued a ForwardHealth ID card in the past, a new one will not be mailed. The applicant can use his or her previously issued ForwardHealth ID card or contact Member Services by calling 1-800-362-3002 to request a new one.

Note: The start and end dates on the temporary card are for 14 calendar days and is not necessarily the end date of coverage because the user should begin using his or her permanent ForwardHealth ID Card as soon as he or she has it.

The user must inform the applicant of the following information:

12.4.8.1.2 Information to Link an Express Enrollment Application with an Apply for Benefits Application

The Eligibility Notice also lists the information needed to link the Express Enrollment application to a new ACCESS AFB application for ongoing BadgerCare Plus, Wisconsin Medicaid, or Family Planning Only Services coverage. This link will pre-populate the new AFB application with some information from the Express Enrollment application (see Section 12.5 Applying for Ongoing Benefits). It is very important that the primary contact person keep the notice to refer to when starting a new AFB application.

12.4.8.2 Application Summary

The Application Summary is available in both English and Spanish. The user should click on the appropriate button to view and print the PDF in the desired language. The user must print the Application Summary and give it to the primary contact person

12.4.9 Application Search Criteria Page

Accessed from the APP Landing Page, the Application Search Criteria page allows the user to search for Express Enrollment applications. The search results will vary depending on the specific search criteria information entered. Only applications from the user’s current location (as selected on the Location Selection page) will be displayed in the search results.

The “Back” button will take the user back to the Landing page.

The “Next” button will take the user to the Application Search Results page.

12.4.9.1 Searching for Express Enrollment applications submitted on or after September 28, 2014

The Search Criteria section allows the user to search for Express Enrollment applications that were submitted on or after September 28, 2014.

Search by individual information

If searching for a particular individual’s application, the user can enter the individual’s  name, gender, date of birth and/or SSN.

Search by county or tribe

If searching for Express Enrollment applications for individuals or households in a specific county or tribe, the user can enter the appropriate information in these fields. The user must select a county from the drop-down menu in order to search for Express Enrollment applications.

Search by type of application

The user must choose at least one type of application (Express Enrollment for BadgerCare Plus and/or Express Enrollment for Family Planning Only Services) in order to search for Express Enrollment applications.

Search by Express Enrollment application status

The user must choose at least one application status in order to search for Express Enrollment applications.

Search by AFB application status

The user must choose at least one status for the AFB application in order to search for Express Enrollment applications.

Search by linking status

The user must choose if the linking status is “Active” or “Expired.”

If the linking status is “Expired,” the applicant must complete a new AFB application without the use of pre-filled information from an Express Enrollment application.

Search by application date range

The user can search using specific begin and/or end dates within a 365 day range. If no begin or end dates are entered, the search results will look at the past 365 days.

Display results

The user must choose to Show Detail Results or Show Total Counts. Show Detail Results will include the following information for each result:

If there are more than 25 search results meeting the entered search criteria and “Show Detailed Results” is selected, the user will receive a message to refine the search criteria to decrease the number of applications meeting those criteria.

“Show Total Counts” will show only the number of individuals that meet the entered search criteria. There is no limit to the number of records shown.

Search by application number

The user can search for a specific application by entering the application number.

12.4.9.2 Searching for Express Enrollment Applications Submitted Prior to September 28, 2014

The Search criteria for PEs submitted prior to September 28, 2014 section allows users to search for Express Enrollment applications that were submitted prior to September 28, 2014.

Search by individual information

If searching for a particular individual, the user can enter the individual’s name, gender, date of birth and/or SSN.

Search by county or tribe

If searching for Express Enrollment applications for individuals or households in a specific county or tribe, the user can enter the appropriate information in these fields. A county must be selected from the drop-down menu in order to search for Express Enrollment applications. A tribe is not required to be selected.

Search by type of application

The user must check “Express Enrollment for Pregnant Women” and/or “Express Enrollment for Children” in order to search for Express Enrollment applications.

Search by application date range

The user can search for Express Enrollment applications within a 90 day range prior to September 28, 2014.

12.4.10 Application Search Results Page

The Application Search Results page displays the list of results from the Application Search Criteria page, along with the criteria used for the search.

12.4.10.1 Search Results for Express Enrollment Applications Submitted On or After September 28, 2014

12.4.10.1.1 Detailed Results

If the user selected Show Detailed Results on the Search Criteria page, the Search Results page shows each application that meets the specified search criteria, listed in alphabetical order by the Express Enrollment applicant’s last name, first name and middle initial. The list also displays the following information:

If there are more than 25 search results meeting the entered search criteria and “Show Detailed Results” is selected, the user will receive a message to refine the search criteria to decrease the number of applications meeting those criteria.

To view or update an Express Enrollment application, the user must click on the “Go” button. If the application chosen has been submitted, the user will be taken to the Results page for that application. If the application is “In Progress,” the user will be taken to the next required page of the application.

12.4.10.1.2 Count Results

If the user selected “Show Total Counts” on the Search Criteria page, the Search Results page will only show the number of individuals and the number of applications that meet the entered search criteria. There is no “Go” button displayed by the count results.

Note: The number of applications may be fewer than the number of individuals because multiple individuals can be on one application.

12.4.10.2 Search Results for Express Enrollment Applications Submitted Prior to September 28, 2014

Each application that meets the specified search criteria is listed in alphabetical order by the Express Enrollment applicant’s last name, first name and middle initial. The list also displays the county the individual lives in and the tribe the individual belongs to, if applicable. The application type and status are also listed.

To view an Express Enrollment application, the user must click on the “Go” button. If the application chosen has been submitted, the user will be taken to the Results page for that application. The user will not be able to update or complete an Express Enrollment application that was started prior to September 28, 2014.

The Back button will take the user back to the Search Criteria page.

12.4.11 Contact for Questions About Using APP

When using ACCESS, an individual can get more information about a given question by clicking on the “Help” button in the upper right of the screen. 

If a user has questions about Express Enrollment, he or she can call the ForwardHealth Portal Helpdesk at 866-908-1363.

12.4.12 ACCESS Training Environment

Users can practice using the Express Enrollment application tool at trn.access.wisconsin.gov. They should click on “login” under “Community Partners” or “Providers/Hospitals,” whichever is appropriate for their organization type. The following credentials should be used to log in:

User ID: sambecket3333

Password: Enter123!

Note: Users should not submit practice Express Enrollment applications on the actual ACCESS website. Users should not submit real Express Enrollment applications on the training ACCESS website.
