9.1 Six-Month Report Form (SMRF)

9.1.1 Six-Month Reporting Introduction

FoodShare members must report current information about their household by completing and submitting a FoodShare Six-Month Report in the sixth month of their certification period (see FOODSHARE WISCONSIN HANDBOOK, SECTION 6.1.2. SIX MONTH REPORTING REQUIREMENT). Some households are required to complete a FoodShare Six-Month Report more frequently if they have received a 36-month certification period. Depending on household circumstances, this could happen in months 6, 12, 18, 24, and 30 of their 36-month certification period.

A paper FoodShare Six-Month Report form and instructions are mailed approximately one month before the form is due. The FoodShare Six-Month Report must be returned by the date on the form for benefits to continue without any delays. The instructions explain how to return the report:

9.1.2 Start an Online Six-Month Report

To report information online, login to ACCESS by selecting Log in on the ACCESS homepage, see SECTION 2.3.5 LOG IN TO ACCESS ACCOUNT. If an ACCESS account has not been set up yet, see SECTION 2.3.4 CREATE AN ACCOUNT.

An Alerts section displays at the top of the account homepage when a FoodShare Six-Month Report form is due. Select the link to begin the online FoodShare Six-Month Report form.

9.1.3 Report Form Overview

A member goes through several sections when completing a Six-Month Report form in ACCESS. The Report form overview page displays all the sections that must be completed. The sections are Your information, People in your household, Household details, Income and benefits, Assets, Bills and Finish and submit. Each section consists of a series of pages, which are based on the household situation and the indicated changes. Information already on file is shown so that changes can be made.

Note Not all of the following pages appear for every Six-Month Report. If a change is indicated, additional pages are scheduled to ask for more information about the change. Not all the pages for a Six-Month Report are shown below. Similar pages are scheduled for Six-Month Report forms and for renewals. For details about specific pages, see SECTION 8.3 RENEW MY BENEFITS PAGES.

9.1.4 Your Information

On the Report form overview page, in the Your information section, select Start. The Your information page displays. 

The member reviews basic information that is currently on file with the agency, including: name, date of birth, SSN, race and ethnicity, marital status, address, and contact information. Select Next.

9.1.4.1 About You Summary

The About you summary page displays. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Update. After making all necessary updates, select Confirm and next.

9.1.4.2 Where You Live Summary

The Where you live summary page displays. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Update. After making all necessary updates, select Confirm and next.

9.1.4.3 Finished Applicant Information Section

After the member completes the Your information section, the You finished the applicant information section page provides an opportunity for the member to review and edit their responses as needed. Once all updates and changes are complete, select Report form overview.

9.1.5 People In Your Household

On the Report form overview page, in the People in your household section, select Start. The People in your household page displays. 

The member reviews information about the people in their household. If a new person joins the household, additional questions may be asked about that person. Select Next.

9.1.5.1 Household Members Summary

The Household members summary page displays. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Edit. After making all necessary updates, select Confirm and next.

9.1.5.2 Household Member Information Summary

The Household member information summary page displays. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Update. After making all necessary updates, select Confirm and next.

9.1.5.3 Finished People In Your Household Section

After the member completes the People in your household section, the You finished the people in your household section page provides an opportunity for the member to review and edit their responses as needed. Once all updates and changes are complete, select Report form overview.

9.1.6 Household Details

On the Report form overview page, in the Household details section, select Start. The Your household details page displays. 

The member reviews information about the people in their household that is currently on file with the agency, including: citizenship, health needs, and legal history. If a new person joins the household, additional questions may be asked about that person. Select Next.

9.1.6.1 Citizenship and Legal History Summary

The Citizenship and legal history summary page displays. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Update. After making all necessary updates, select Confirm and next.

9.1.6.2 Finished Household Details Section

After the member completes the Household details section, the You finished the household details section page provides an opportunity for the member to review and edit their responses as needed. Once all updates and changes are complete, select Report form overview.

9.1.7 Income and Benefits

On the Report form overview page, in the Income and benefits section, select Start. The Your household details page displays. 

The member reviews information that is currently on file with the agency about the household’s income and benefits, each person’s jobs, anything they do to make money, and money from different programs and benefits (for example, child support, pension payments, veterans benefits and more). If a new person joins the household, additional questions may be asked about that person. Select Next.

9.1.7.1 Work Activities Summary

_____’s work activities summary page displays for each member that has a work activity. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Edit. After making all necessary updates, select Confirm and next.

9.1.7.2 Other Income Summary

_____’s other income summary page displays for each member that has other income. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Edit. After making all necessary updates, select Confirm and next.

9.1.7.3 Finished Income and Benefits Section

After the member completes the Income and benefits section, the You finished the income and benefits section page provides an opportunity for the member to review and edit their responses as needed. Once all updates and changes are complete, select Report form overview.

9.1.8 Assets

On the Report form overview page, in the Assets section, select Start. The Assets page displays. 

The member reviews information that is currently on file with the agency about the household’s lottery and gambling winnings. If a new person joins the household, additional questions may be asked about that person. Select Next.

9.1.8.1 Lottery and Gambling Winnings

The Lottery and gambling winnings page displays. After completing the page, select Next.

9.1.8.2 Finished Assets Section

After the member completes the Assets section, the You finished the assets section page provides an opportunity for the member to review and edit their responses as needed. Once all updates and changes are complete, select Report form overview.

9.1.9 Bills

On the Report form overview page, in the Bills section, select Start. The Bills page displays. 

The member reviews information that is currently on file with the agency about the household’s payments and other bills. If a new person joins the household, additional questions may be asked about that person. Select Next.

9.1.9.1 Housing and Utility Bills Summary

_____’s housing and utility bills summary page displays for each member that has housing and utility bills. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Edit. After making all necessary updates, select Confirm and next.

9.1.9.2 Other Bills Summary

Other bills summary page displays for each member that has other bills. 

This page allows the member to review the information that is currently on file with the agency. To make changes, select Edit. After making all necessary updates, select Confirm and next.

9.1.9.3 Finished Bills Section

After the member completes the Bills section, the You finished the bills section page provides an opportunity for the member to review and edit their responses as needed. Once all updates and changes are complete, select Report form overview.

9.1.10 Finish and Submit

After the member completes all the updates, the Report form overview page provides an opportunity for the member to review and edit the responses as needed. Once all updates and changes are complete, select Start.

9.1.11 Sign and Submit

A member needs to sign their Six-Month Report prior to submitting it. Once the Six-Month Report is submitted, it is processed by an agency. Sometimes, a member needs to provide verification of the changes to the agency. The member is notified about required verification and when it is due. Without this verification, the Six-Month Report cannot be completed, and benefits may end.

9.1.12 Next Steps

From the Thank you for submitting your report form page, members can submit verification of the changes, view and print a summary of the submitted changes, and view their agency's contact information. The tracking number of the report form displays at the top of the page.
