7.1.1 Six Month Reporting Introduction

You may be required to complete a mail-in Six-Month Report form (SMRF) for Child Care or FoodShare. If you are, you will get a reminder letter that the form is coming.

 

Then the Six-Month Report form and instructions will be mailed to you one month before the form is due. The instructions you get with the form will explain how to fill it out and where to mail it, if you choose to return it by mail.

 

You may also submit your SMRF online in ACCESS.

To begin your SMRF you need to log in to your ACCESS account.   See 1.3 Ongoing User Login.

 

7.1.1.1 MyACCESS

To begin your SMRF, click on the link under “Six Month Report due for”.  For example this customer would click on “Click for Food Share”.  If there is a SMRF due for another program, like Child Care, there will be a link available for that program as well.

To submit your SMRF online, read the Online Six Month Reporting Overview page and click next.

 

After clicking Next, information that is already known will be shown on a series of pages so that you can make any changes or updates.

 

 

 

7.1.1.3 SMRF Pages

As you do your SMRF online, you will go through a series of sections.  These sections are People, Benefits, Job Income, Other Income, Bills and Submit.  

 

Pages for you to enter information on are organized into these sections.

Information that is already known will be shown on this series of pages so that you can make any changes or updates.

 

As you are doing your SMRF, you will see a Comments button on the right side of the pages.  At any time, you may enter any comments you have related to the information you have provided on a page.

 

Following are many (but not all) of the pages you will see when completing your SMRF in ACCESS.   Note that not all pages appear for every SMRF – you will only be asked to fill out the pages that you need based on your case situation and the programs you have requested.  Also note that whenever you indicate that there is a change, more pages will come up to ask you for more information about those changes.      

 

Not all pages are displayed in this handbook.

Once you finish your SMRF and submit it, it will be processed by an agency.  Sometimes, you'll need to mail, fax or bring proof to your agency.  You will be notified by mail about proof that is needed and when it is due.

 

Without this proof, your SMRF cannot be finished and your benefits may end.
