Eligibility for FPOSFamily Planning Only Services - This plan provides certain services and supplies to people who are of childbearing or reproductive age to prevent unplanned pregnancies. begins on the first of the month of applicationA request for BadgerCare Plus coverage. The request must be on the Department's application or registration form and must contain name, address, and a valid signature. The applicant must submit a signed and completed application form to complete the application process., if all non-financial (see Section 40.4 Nonfinancial Requirements) and financial (see Section 40.5 Financial Requirements) eligibility requirements are met. FPOS may be backdated up to three months from the month of application.
