6.4 Fax, Mail, or Drop-off Documents

Applicants and members can also submit documents by faxing, mailing, or dropping-off the documents at their agency.

The More ways to submit documents page allows applicants and members to choose how they want to submit their documents.

If the applicant or member has more than one application, the [Case/Tracking] number drop-down displays and the applicant or member selects a case or tracking number.

Applicants or members should do the following:

  1. Prepare the Document Tracking Sheet (DTS) and place it at the top of their documents.
  2. Write their phone number and the number of pages being submitted in the space provided on the DTS. Make sure to count the coversheet in the number of pages.
  3. Make copies of the original documents such as ID, birth certificate, and pay stubs. Do not mail in the original copy unless the case worker specifically requests it.

The Document Tracking Sheet (DTS) is sent to applicants and members by mail and is available to print in ACCESS. To print from ACCESS, select either Fax, Mail, or Drop-off at agency, then select Download and print Document Tracking Sheet (PDF).

6.4.1 Submitting documents by fax

Fax the documents to the number provided on the DTS or in ACCESS. For more information on Wisconsin’s income maintenance and Tribal agencies, select here

Note Only one-sided documents can be faxed.

6.4.2 Submitting documents by mail

Address the envelope to the agency address listed in ACCESS. For more information on Wisconsin’s income maintenance and Tribal agencies, select here.

6.4.3 Submitting documents by drop-off

Drop-off the documents and DTS at the agency address listed in ACCESS. Select the Agency hours and information link to find a time to drop-off the documents. For more information on Wisconsin’s income maintenance and Tribal agencies, select here
