To start a food benefits replacement request, log into the primary person's or the primary person's spouse's ACCESS account (see SECTION 2.3.5 LOG IN TO ACCESS ACCOUNT). (Only the accounts of the primary person or the primary person's spouse can be used to complete the request.)

Select Food replacement request under Manage Benefits on the account homepage. From the account homepage, the member can also select Food replacement request from the “My benefits” section and select FoodShare.
| Note | The Food replacement request link is only available to members that are either current FoodShare recipients or whose benefits have been recently closed. |

The Your Food Benefits Replacement Request page displays. All fields with a red asterisk must be completed. After completing the page select Submit.

The Submit your request page displays. Review the information, select the checkbox and electronically sign the request. Select Submit to submit the request.

Once the Food Benefits Replacement Request is submitted, the Your request has been submitted page displays. The page displays a tracking number, which allows the member to track their request. Members can use the document upload process to upload verification when requesting food benefits replacement or after receiving a notice that they are required to verify the misfortune. For more information on submitting documents, see SECTION 2.4.4 SUBMIT DOCUMENTSj.

If the member selects Download summary (PDF) the Food Benefits Replacement Request Summary PDF displays. From here the member can view, save, or print the summary of the electronic submission.
The member can track their Food Benefits Replacement Request from the ACCESS account homepage in My recent activity. Members can see the status of their request and are able to view the summary PDF for 90 days.
